This form is for parents or professional’s wishing to refer a child for an individual consult to determine the best approach for providing assistance to the child. This forms is used for all children and for those who may or may not have NDIS funding.
Carer Assessment Referral Form
This form is for referring agencies who wish for Deadly Descendants to complete a general Foster, Kinship or Guardianship Step-By-Step Assessment. Please note this referral has seven parts and additional information will be requested through email once referral has been completed.
School Referral Form
This form is for the Principal or someone in a leadership role at your school who would like to know more. Click this link to fill in a quick referral and Deadly Descendants will give you a complementary 20 minute discovery call.
Client Registration Intake Form
This is our new online intake form for new clients. This form is completed after your referral has been accepted and you have been offered an assessment/intake session. Please complete this form at least 48 hours BEFORE you attend your first session.
